Born. Your gender is not "chosen", like an outfit or a haircut; it's acknowledged.

I suppose an exception could be made for hermaphrodites, since a decision needs to be made there, but they made up only a tiny fraction of transsexuals.

I personally would only use the term “assigned male/female” at birth for people born a Hermaphrodite or intersex with birth sets of male or female reproductive organs.

I would only use the term “assigned male at birth” or “assigned female at birth” for only people who were born with male and female sexual reproductive organs

If a person is born with only male sexual organs than I would say that person is born male. Not assigned. If a person is born with only female sexual organs they are born female. Not assigned. Doctors don’t give out genitalia to a person. A person genitalia is set before they come out of the womb.

Assigned, since the doctor doesn't actually check everything like chromosomes or internal organs, they just look at the external genitals.
