I do, but most people in real life don't ask the kinds of questions that they ask online. People that tend to get their feelings hurt by blunt commentary online generally ask extreme questions and then when they get honest answers they can't take the truth. In person, many sensitive/emotional people avoid topics that are controversial or that could lead backlash or personal criticism, so as a result they don't get as many blunt answers that crush their ego. They may be thinking about all sorts of things, but they will not ask other people. That's why they go online with all their crazy, because people online don't know them in person. If you prompt me with the same sort of questions in person, you will get the same sort of blunt response and likely get far more detail. There are character limits on these sorts of sites, so you can only say so much.
In a public speaking course where we had to write persuasive and argumentative speeches and debate I actual got an award for being the most controversial speaker. :D

in "my real life" I never heard of so many things like incels, manosphere, "body counts" and the endless types of fetishes and what not...

You are only free to speak unfiltered when you no longer care of the consequences. Either this means you're already stinking rich like Trump or Musk. Or you've accepted your life is total shit and people already have zero respect for you.
Speaking unfiltered gets you fired or a fist in the face. People have long memories and when you need them they remember the shit you said.
I wouldn't keep my job for long if I did that.

I am very blunt but it does depend on my location or position. For example I am in a vulnerable position work wise and know my boss opposes the political party I support. So at work I avoid politics and would not be open about it.
This sawno alias I specifically created so I can be maximum honest. So here you will get the most honest most blunt version of me. Where in other communities I may have to be a more neutral role.
Any version of me is me though. I don't fake anything. But at some places I can be open and in others I interact tactfully.
What you see here from me is what you get face to face. I may be a tiny bit blunt and direct in my communication style.
