I see you're from Slovenia. Technically, you'd need one for each country. If you're traveling through the Schengen Area that counts as one country. And it would need to be authorized by a notary.
But double check, because some countries may not require this. I know we had to do this when my mom was taking me to Canada, but I don't recall ever doing it when we flew to Poland.
Also, I visited my aunt in Poland and we took a bus to Greece, and there was a school group traveling with their instructor. I don't recall any of them having a permission slip from their parents. I don't think they care so much in those countries. Best to check with the embassies of each country or an immigration lawyer. I doubt anyone on GAG can give you an accurate answer.
Make sure you all have passports. That's the most important thing.

You guys are bugging plenty of families go on holiday with friends kids aslong as u have the passport thats essently the proof of being allowed u can do extra stuff but its not required lol
Thinking of it now, I think this paper might only be required if one parent is traveling with their kid. Because I know some countries especially the U. S. and Canada started doing this, because there were too many issues with one parent taking their kid against their spouses will or court orders and fleeing the country.
have the parent sign over permission to travel together in handwriting.
