I don’t regret the decision as we are super happy together, but I do still feel guilt a few years later. He stopped talking to me after I told him.

Yea you pretty much betrayed your friend it's just about that simple but it's also too late to take it back and fix things so your bed is made and all you can do is accept you gained a wife and lost a friend and deal with the fact you made that decision and did something wrong. If your friend didn't come up and say hey I know we didn't work out together but I think the two of you would be grate for each other. then yes it looks bad to be dating their ex. But I would find it hard to if the person I'm dating was a friend first cause it means we were already from the start.
