Girlfriend had a relapse after being clean for 18 months, how can I help her?

I assume we are talking about drugs.

Drug addicts always relapse. There are a FEW cases where they kick the habit but tons more cases where they relapse, some end up overdosing and dying.

They have to do this themselves. You can't HELP them. Don't do drugs or drink around them or take them places that will tempt them, that is about it.

Bigger question is why don't you think about finding another, better situation with a girl who doesn't have this huge, heavy gorilla on her back?

That is hard.
Sady like any addiction it is up to the addict themselves to make that decision that they want to stay clean.
You can be there for support, but until she has had enough there isn't much else you can do.
Sadly, many addicts relapse a few times before they can finally stop for good.
The bodies drive for the drug over comes everything else.
Nothing else matters except the next high which is a really tough battle.
We have had many calls for addicts that have overdosed after coming out of rehab or started using again.
The body will lose its tolerance for the drug, but they go back to the dose they were using before and then we get called.
It's not like there is any sort of purity standards either.
It's sad to watch someone struggle through that.
If she is ready to try again, see if there are programs that you can attend to offer suggestions of what you can do to help her out.

the fact she confessed is actually a blessing, she is aware that she's in a dire situation, see if you can get her into a treatment center even one provided by the state or funded by the state
