My sister doesn't do much , she hasn't had a job in 15 years, takes welfare , doesn't work and last year she was almost found dead in her room , she refuses help but why

She's probably sitting in the house all day doing nothing which causes her to ruminate on whatever is troubling her mind and she probably has anhedomia which makes her feel a lack of pleasure. She has to get out the house and do something. Just try to get her to do volunteer work that is easy for her to handle. The best way to fix what's wrong with her is with work or with something to do. But she's older and it's probably going to be hard and maybe impossible for her to change. Maybe for someone in their 20s you can get them to change, but for her, she might be stuck that way. Just get her something easy to handle even if it's just picking up garbage in the park. She probably can't handle the jobs she used to work
because she's been out of work a long time. And employers will be hesitant to hire someone who has been unemployed for 15 years. So the only way is to get more schooling or work for free until her labor is worth a decent wage.

If she's not willing to help herself or do anything about it, there's not much anyone can do for her. She has to decide to overcome her challenge or remain stagnant where she is in life. We are what we overcome. She's feeling like this way and she doesn't go see a therapist or do anything for herself. there's not much you can do to change her. I know you don't want to give up, but you can't make her do anything it's up to her to decide she wants a better life for herself.
