Location and event. If you're somewhere people go to socialise then it's generally OK to approach, just don't be that creepy guy who's working the room.

The question that runs through every woman's head on an approach is "Is this guy safe?" It's not normal to stop people on the street. If someone is waiting for a bus and you make a bee line to them for their number it raises a big red flag. If we're in a bar/club and you walk up with a cheesy chat up line you found online we will vomit.
