I need to find a way to get my girlfriend to lose weight. How do I handle this without ruining my relationship?

I'll try to keep this brief.

I love my girlfriend, I don't want to breakup, but she has always been a bit overweight. It affects her confidence and is starting to affect my attraction to her. How do I tell her all of this without losing her.

Been dating a year, she moved in after 6 months. We never had a ton of sex, but twice a week at first. Then once a week. Now it's like once every 2 weeks a year in, and steadily declining. I love her and we are very affectionate, but she has no sexual confidence. I always initiate. Sex is on me, it doesn't feel very relaxing or desirable, so it's been hard to get in the mood.

She has a pretty face, but she comes home every night and puts on a giant t-shirt, puts her hair in a sumo-style top bun, and takes off all makeup. I have tried to tell her I don't like it, but she gets upset, or doesn't take it seriously.

I don't want to make her feel worse about herself. But our lack of sex is starting to affect her and me to be honest, and I can't keep blaming it on myself. I'm not expecting her to be rail thin, but maybe lose like 20-30 lbs.

I'm down with going to the gym with her, I also need to workout more. I'm not saying I'm in perfect shape. But I wear normal clothes the whole day, and I pay attention to my weight. I'm 6'1 and if I go over 200 I start changing my habits. She is 5'0 and probably close to my weight.

She has started to question me on our lack of sex, and I'm running out of ways to say things without hurting her. I really need some help here. Serious answers only please.
