He has low self-esteem. There is a great chance he was bullied or gone through psychological trauma at some point that resulted in him looking down at himself. The best thing you can do for him at this point is to compliment him whenever you can, and invite to meet up with him if the two of you are in close proximity to each other. If that isn't an option, then invite him to web cam with you eventually.
