What does it mean when a man asks how are you feeling?

I would say the chance is there and it's something you can suppose, depending on his tone of voice. Because if asked it in a normal or steady voice its more likely than him asking it jokingly.
Another argument is that many guys normally aren't interested in talking about feelings

So asking about how you feel increases the chances that he really is interested to know.

Depends on the context and the relationship but he's at least being cordial.

A lot of people ask this almost as a hello but only a close friend or lover really wants to know the answer.
