Are guys as romantic in real life as they are in movies? I. e. saying sweet things, gifts, special moments, making her feel special (surprising her)?

None of the options fit, in my opinion. It depends on the guy, his level of emotional intelligence, how well he understands you and your needs within the relationship, how much he understands the importance of romance in keeping a relationship/marriage healthy and happy, etc. I don't think it comes naturally to very many men, especially if they didn't see their fathers doing romantic things for their mothers growing up, but it can definitely be learned and worked towards. I've only been in two relationships, but neither of them started off romantic and they had no idea what I was asking for when I expressed a desire for more romance. Both of them had parents who were divorced and had no example of healthy loving relationships the way I did, where my father did spontaneous and romantic thing for my mother all the time.
Depends on the guy. The guy I’m currently seeing surprised me with flowers on our way to our date for my birthday.
a lot of guys are like this but it's happening less and less since today they'd be called a simp.

No, sadly, most men are clueless about romance.
