i have been depressed about my life for the last 3 years because of my dads death he died in 2016.
i dont brush my teeth i have a cavity plus i have plaque.
i quit my job 3 months ago i have not wanted to get a new on yet.
i have not taken a shower in at least 4 days.
i drink too much soda.
i never got my drivers drivers license.
i need to get my eyes rechecked

This sounds like a job for an accountability partner. Do you have friends or roommates at all?
In the meantime though to hold yourself accountable, you should write out a daily routine, a list of things you need to do daily in regards to hygiene, personal health, and so forth. It should be a list that you can sign off on when something is completed. Start small (say, three or four things) and go from there. Like this...
Maybe include a weekly task too, like cleaning the bathroom.

In the height of my depression on my best day I could brush my teeth by having a toothbrush, toothpaste, a cup and some bottles of water next to my bed.
Showering is more difficult unfortunately you’ll have to try and motivate yourself and get back to a place mentally where you can do that.
I also used to drink too much soda and it would make me sick but you just have to try and substitute with water and always have water bottles near you around the house.
I don’t have my drivers license yet either but I don’t think it’s a big deal honestly. You’ll get there when you get there.
It’s hard because I don’t have a lot of answers for you since I’m still struggling with milder (but still persistent) depression and anxiety but just know I’m on your side. I’m sorry I can’t help you more and I know it’s difficult not to be hard on yourself. I understand you completely. I also hope you can somehow find access to therapy.
There is no magic jump start. It's will. Hopefully once you start taking steps in the right direction it will ease the depression and have a snowball effect
