i know, it sucks to care about someone who doesn't want you, I've been there more times, but in this case you need to do very hard thing and that is go against your feelings, in this case that is actually the right thing...
I would say no. If he hurt you then he doesn't deserve a happy birthday. So basically he gets to hurt you and gets a free happy birthday? You won't gain anything, if I were you just ignore him.
