How to eat healthy?

I feel like I haven't been eating lately and feeling like I can't concentrate on studying and feel week and stuff. So I want to change that and start eating healthier (! i do workout) I want to be healthy and loose weight. What I usually have for breakfast would be donots or muffins and hot chocolate milk only because I don't have time to eat breakfast at home. So how can I eat healthier, like what should I eat for breakfast, lunch and dinner?
