I imagine they used the same thing as they use today when they can't afford modern products. Just because something exists doesn't mean everyone uses it - either because it's not available or because they can't afford it. Even in a modern first world country like the US, not all women use modern products. I know from first hand experience with someone I dated.
People shouldn't take things for granted, not even food, water and shelter.
