I think Communism had that one figured out. They woud refuse to let people sleep (for weeks) tie you to a poll and then push you down a over 100m long stair, lay you on the floor naked while guards hold your arms and legs, and then one of the guards would put one of hees feet on your balls, applying more and more weight.
