Depends on what they are referring to. If they say you're thick headed, that's totally an insult. If they mean your weight and not your overall shape, also an insult. Thick isn't really a solid insult, but not everyone would take it so well.

In regards to looks... Some people do. Look up THICC on UrbanDictionary. Though even if someone meant it that way, which could be considered a compliment... should you really be dating someone that thinks THICC is a good way to compliment someone? I don't think most people talk that way, though (even teens specifically).
