There's really no excuse to still be surprised about hearing this. Not that it matters since this question is for other people like me anyway.
If you have an internal monologue but you wanna answer anyway, fine. But don't broadcast your stupidity to the entire damn website pls.
I often find that when talking to people, I spend a lot of time in "translation" mode to make doubly sure I'm using the correct words for the correct concepts. So often whenever I attempt to simply talk, there's a horrendous misfire and I then have to go back and correct myself because I ended up literally saying the exact opposite of what I meant.

I never had that problem. Try expanding your vocabulary.
It's not a vocabulary problem.
It has to be if you can't associate the meaning of a word with the correct word. If you are a visual thinker you should be able to associate your thoughts/images with meaning. So in order to transition that into any language, it's just about learning the meaning of particular words in whatever language you are working in. Thus you have to study the vocabulary so that you know the absolute meaning. Personally I tend to use something like Anki which is a open source repetition flashcard program.
