What mistake can you make in life that has the most severe consequences?

In my case, probably not a "mistake" per se, just stupid errors of judgement, and maybe somewhat the fault of the kind of family I was born into. As a result of my lack of foresight, not seeing what the future held for me, I did not concentrate on 'learning', and MAYBE like many other young guys, did not develop the skills and abilities I needed to compete in the workplace successfully.
When I left my birthplace of origin and moved to California, I had a HUGE opportunity to capitalize on the skills that I SHOULD have acquired early on, and as a result I did not/could not take advantage of the employment opportunities here in CA with the largest corporations on the planet. The opportunities were there, but I did not have the learning/skills/maybe the intellect to take advantage of those wonderful opportunities
For me, I suppose I can call that a mistake or mistakes that had severe consequences for me later on in the work place.
In spite of that, I did 'recover enough to feel that I made up for the lack of those skills and to a degree, minimized the consequences.
Breaking the law , and this is not always clear cut as one may think , selecting the wrong employment option , and getting caught ( possibly married ) to a partner where it was never going to work , merge all three and you have utter disaster , then you face the severity of actions you yourself created.
Usually cheating, whether that be on a partner or on a test. If you are caught cheating in a relationship, you can lose that relationship as well as your entire community as they shun you and push you away for knowing that you would commit such a massive act of betrayal that they can no longer trust you. For tests, like say the bar exam, or the test you have to take to get your CPA, or your engineering license, if you are caught cheating on those tests you will lose those certifications, lose your job, potentially your home if you’re not careful.
I would say getting pregnant with a man who isn't loyal to you and leaves you in 5-10 years for another woman. My cousin has a family acquaintance who has a lot of money and he's abandoned 3 wives so far. Also dating a bad person cause he or she is rich or good looking. I've seen people do that and their lives are so miserable, I think they would be happier in a jail cell.

Top of my head.
Having the wrong parents. You really need to have a financially well off and supportive mother and father. Having access to good healthcare and a good education is the best way to have a happy, productive life. Growing up in poverty in a single parent household is a sure fire way to be poor all your life.
Taking drugs! Just don't do it! Say NO! I've never been an addict myself, but I've seen what it does to people, and it's absolutely horrendous!
One of the most severe mistakes one can make in life is harming or causing harm to others, whether intentionally or unintentionally. This can have long-lasting consequences, impacting not only the individuals directly involved but also their families, communities, and even society as a whole. It can lead to legal repercussions, damaged relationships, emotional trauma, and profound regret.
If you are man: marrying the wrong woman or getting her pregnant.
If you are a woman: getting pregnant outside of wedlock or engaging in sex work.
Either: getting convicted of a felony.
Marrying the wrong person and children being involved
Something illegal that can put you in a prison for a very long time.
Going to prison and losing family over it it’s never happened but yeah that would be awful
getting reputation destroyed
that could follow person to the grave
For us normal Americans, not paying taxes would result in severe consequences. Only the rich people can get away with doing that
Getting pregnant and keeping the child even though you don’t want it. You’re screwing your own life, and harming an innocent child who didn’t ask to be born.
I responded to a personal ad. She wanted to hookup. In retrospect, it was too good to be true. "She" was a cop. I was arrested and jailed.
