If you remove someone from having access to a child's medical history/info are they notified by the doctor's office (HIPAA)?

If you remove someone as an emergency contact or someone who can know information regarding your child's medical history/care, does that person get notified they were removed and are no longer approved to know these things or do you just tell them yourself or let them figure it out?

I updated my child's HIPAA info and I removed a problematic relative from having access to their info. This relative used to be an approved contact and someone who was permitted to have information shared with them regarding my child. Thanks to recent drama and toxic behavior, I made the decision to have them removed and no longer allow them these permissions.

Does the office call or text the person to tell them they've been removed or is that something you can personally do if you choose to? I don't plan on telling them honestly. They'll find out if they ever try to seek info regarding my child. I just know they'll cause me more issues if they find out I removed them.
