How do I go about getting copies of my medical record from when I was a minor?

What all do they include in a medical record? Are notes from therapy sessions available?

You'll have to ask your personal doctor for some medical release forms, leave them blank and sign them before you turn them back in. Give them the information to collect from that doctor you were treated by and have him/her mail them to your personal physician. You should be able to get copies from your own doctor after the other doctor fills them out and mails them back.
That's the way I have done it in the past.

I'm not sure about there but in Canada all I have to do is ask my doctor.
