Can pharmacists look back at your medication history?

Absolutely they can here in the US all states are connected through a prescription monitoring database. This is done to keep people from doctor shopping for prescriptions. Also some states doctors and rx can pull what you purchased at a dispensary.
If you go to the same pharmacist I would think so. Could be different in Australia to the US. With escripts it is easy for a pharmacist to keep a complete record of what you have been prescribed.
We also have option of opting in to have medical records available to all practitioners. I think that is beneficial to me so I have opted in..
Yes, I believe in the US, there is a database that they all share for controlled substances. The rest of your medications would only be accessible by whatever pharmacy you filled a particular prescription at.

depends on how old. If it is from 2002 then they probably don't have it anymore.
