5 dI grew up in era where we boys teased each other so often that what counts as an insult has to be quite severe, and undeniably malicious. It's very rare in my experience according to my standards, although my idea of a proper insult that you have to be willing to fight to the death for it.

5 dIn person by my Uncle. I think I told you this once ha. We got past it though. He had some weird outburst and insulted me in front of my family at a get together and it was so out of character for him. Nonetheless it was brutal, out of nowhere and unwarranted. Sometimes that generation forgets, my generation is 40+ and not children anymore.
I asked him nicely a few times to take his tone down a notch twice, and when he tripled down, I did something I'm not used to. That being yelling at my elders.
He was confused at the idea that actually defended myself because of their weird, "I'm older than you and you just have to take it and shut up" old school ways. Well that got him to snap out of it and I told him take a walk and cool off like a child.
A week later I realized he wasn't going to call and apologize. That's their generation (in my family) so I called him to check in. He was never like this with me. As an uncle myself now, without kids, I take being an Uncle to heart. I could never imagine being so cruel to my nephews. Let alone if they were my age lol. He picked up my call expecting me to apologize but I didn't.
I asked him to hear me out and wanted to uncover what was going on to cause him to have a random unwarranted outburst towards me. He didn't know and couldn't really remember why he was mad.
After a bit, on the phone something felt off so I just apologized because it seemed important to him and then of course he immediately apologized as well. He told me he had a random doctors appointment and I asked him to talk about why he doesn't remember or what he was mad about with me that day.
My Aunt actually forced him to bring it up. Dementia ran on his side of the family and the doctor is now keeping a look out for it. My uncle might be in the beginning stages.
So the moral of the story here is try not to be angry with your loved ones even when stupid arguments happened. It made me realize how life's short and we need to enjoy our time with our family while we can.
He seems ok so far but it really made me feel bad yelling back at him. I tried but after a few min of finger pointing it got difficult. If I would have known maybe he was unwell I would have handled it differently.

4 dOnline, almost always, if we’re talking about true insults. My friends are the kind who roast each other in good fun.
I am a fright to look at in person. People do not f*ck with me unless they are looking for real trouble. It’s strange because i don’t see in the mirror whatever they are seeing. I did have one little guy in South Lake Tahoe try to insult me. I am fairly certain it was born of insecurity because his initial reaction to me was the same as everyone else’s. Plus he had a young lady with him. He couldn’t let her see him become unsettled by the presence of a large, intimidating man. So i smiled and laughed, and he relaxed. We ended up having a delightful conversation thereafter.
There was a guy more recently who attempted to insult and intimidate me, but he was a moron. So he mostly just made himself look like an *sshole. It was also a case of Dr. Jekyll and Mr. Hyde with him. When he came into the bar, he was sober and chill. Once his buzz set in, he became a raging prick.10 Reply
5 dWhen it had hurt? Probably when... that's not even an insult, but it is the last thing i can remember that i am still pissed about. When those from my family who were religious would accuse me of ogling myself when i did not. See, i never cared about appearances. All i ever tried to do is not be weak and not look gay. I am the only one from my family who would never even use combs. I always preferred to just wash my hair and let it sit however nature puts it when it is dry. I always avoided too colorful clothes to minimize attention. But it just so happened that as a teenager i turned the tallest and physically capable of lifting heaviest in my family. I only exercised so that nobody can pick on me, to reduce attention, not increase it. My mom wanted me to be saved for her array of church hoe girls and so to prevent anything i might have with unapproved girls she oppressed me, for example just accused me out of nowhere of checking my own muscles out. Even when i would be disinfecting a wound she would accuse me of finding an excuse to look at my muscles. Daily shit of this sort. Jelous brothers would do that too. And i am pissed about it. I do not get pissed about regular name calling.

Indirectly online? I'd say every other day, when someone posts something insulting the beliefs I subscribe to.
"Personally" online? I dunno... maybe... Tuesday? Or not, I don't know. All I know is that I called someone on something here, they didn't like that, and then the next thing I know, I have about 10 notifications, all from the same person on the same post. I skimmed through it and just thought "boring... boring... boring, lah-dee-daaahhh..." and then asked if they had a shorter version. I dunno, there might have been a legitimate insult in there, I didn't read the whole thing.

5 dIt was from a friend who I am no longer a friend with. She seemed to think I was too old to be interested in sex. That I was "over that." And she was maybe two years younger than me. But was married. She did a host of annoying things, not just that insult. It all added up to us being unsuited to each other.

The third part of your question is a bit more difficult to answer because I’m very sensitive, so I can take insults to heart and get hurt easily. But I try to think about who the words are coming from. If it’s someone whose opinion doesn’t or shouldn’t matter to me and who doesn’t play any role in my life, I usually just ignore it.
