In my opinion, backhanded remarks can be worse than direct slang words (such as m**cker, or a$$hole). For example, when you say to someone "you look good for your age", that's not a complimented because you're implying that someone's old.
Also insults that have to do with mental health, calling someone crazy, nuts, mental, looney or psycho when they're not because that challenges their reason, which is a basic human capacity and credibility
