Not st all. Facebook is intended as a means of connecting with people. A friendly colleague is a perfectly good choice for that, though LinkedIn is the more typical way of staying connected with wirk colleagues.

Personally I wouldn't. I have lots of FB friends that are people I've met at work. If you're friends at work, I don't see anything wrong with it. If you're not sure how they would react, ask them in person if it's okay.
