To me, it's about who you turn to for support in your hardest times. When you are feeling your worst (not physically) and want someone to comfort you, or when you are struggling with what to do in a difficult situation, and so on and so forth. Those are things you go to your spouse for. Maybe not every time, maybe almost never for certain topics, but in general, you go to your spouse. If you stop going to your spouse and instead choose someone else of the opposite sex almost every time, that is emotional infidelity.
