Women have vaginas and its associated plumbing, and men have penises (with which they do most of their thinking). The two sexes are required in order for the species to reproduce, and they are not capable of developing into the sex they were not born as.

According to the Mayo clinic, "Klinefelter syndrome is a genetic condition affecting males, and it often isn't diagnosed until adulthood". They're still male, even though they have this disorder. Even so, the chromosomes are either X or Y, and not W or Z. There are only males, females, and (very rare) chromosomal abnormalities. Two sexes, two genders. That's it. Eunuchs don't count.

@suzzieQ In my native language they are no words for gender, both words are sex (køn). What you call gender is kønsidentitet, meaning sex identity. I suppose this is what you mean when you say gender.
However, sex is not assigned at birth, it's observed based on physiological characteristics. The very few intersex people can be harder to identify. But only approximately 1 in 2,000 individuals are born with intersex traits, which equates to about 0.05% of the population.
Identity is a spectrum since you can identify as anything, but man and woman are defined by sex and sex is not a spectrum. You do have a spectrum of sex characteristics, meaning a spectrum within males and females with some overlaps, but is not the sexes but sex characteristics.

It has only been recently that a small segment of society hijacked the term "gender" and turned it into a social construct with definitions that are so absurd that they have rendered the term meaningless. So yes, there are social definitions, but the vast majority of people will ignore them and continue to define the term as it has always been intended, as a synonym to sex.
