How do you think the world would be different if the Roman empire never fell, but flourished and further expanded?

It is already a science fiction scenario to imagine a much more prosperous future for Rome than it was.
Although the limit of that empire in my opinion, if it had not had that end, would have been the whole world. Even today Rome has conquered humanity through their innovations, principles, wisdom and inventions.

I’d read a book once which answered that question (Roma Aeterna), but what I’m really curious about is how our world would be different if the Republic had never become an empire.
