What does the phrase "running on empty" mean to you?

I meant this in trms of your life, not really your car.

It is a term that I am very familiar with. During a marathon after about 20 miles people have consumed all the glycogen in their muscles. A feeling of extreme fatigue soon follows. The race becomes more of a mental exercise than an athletic contest. The last 4 or 5 miles can be excruciating.

Worn out, exhausted, in need of sustenance. Or the last 20 miles or so between your car telling you it's empty and the fuel finally running out.
