For mental health it depends on the review for the patient that spent time under care. If the patient had an amazing outcome that was life changing that would be less of an issue. Laws are different between the US and Canada. The US has a lot of different options and different laws for each state. Checking out your local facilities website might give you a start in determining the requirements. If the website does not provide the information, email the facility or stop in for a visit.
I would encourage you to seek the counsel of the Lord early on while you still have much time at hand.
Below is some info from wikipedia.
The US, Ukraine, Russia and Georgia have the most liberal laws in the world, allowing commercial surrogacy, including for foreigners.
Gestational surrogates may struggle with postpartum depression and issues with relinquishing the child to their intended parents.[27] Immediate postpartum depression has been observed in gestational surrogates at a rate of 0-20%. Some surrogates report negative feelings with relinquishing rights to the child immediately after birth, but most negative feelings resolve after some time.
Among gestational surrogacy arrangements, between 19%-33% of gestational surrogates will successfully become pregnant from an embryo transfer. Of these cases, 30-70% will successfully allow the intended parent (s) to become parent (s) of the resulting child.[9]

I don't think so. A kid and surrogacy both are very pricey proposals.
Doesn't hurt to try girl. I feel like if you are not a threat to yourself or others then hopefully they should be more empathetic and understanding.
It cost 100,000-200,000 and yes your history will be taken into consideration.
