One of our hardest working employee is acting different she's coming in late going often bathroom always on her phone watching movies and has her headphones in she’s always on her phone

Only if it’s affecting you in some way, like it’s messing with her ability to get work done and as a result you’re having to pick up her slack and/or it’s putting more work onto you. Otherwise no, I’d say to mind your business.
