Not necessarily in Australia. Pasta is a common meal for me so bread is a bit of an overload. In the home it is more of a grace note to a special meal like at Christmas. I doubt anyone slaps down a slice of manufactured bread anymore but my parents did.
At restaurants bread rolls are commonly served early before the main - probably because they are conscious of people like me who will be snarling "I ordered 30 minutes ago and I'm HUNGRY. Where is it?". A warm bread roll will give me patience.
US - Bread is not part of every meal here, but is common enough to see. It is not the same as I'm assuming you are talking about, a baguette. Here it is more commonly seen as the commercially made pre-sliced white bread, or dinner rolls. My wife is Italian and occasionally makes a loaf of bread to tear pieces off of to dip in olive oil with parmesan cheese and herbs.

Sometimes, but it depends on what the meal is. Not for every meal of course. Sometimes it can be a simple eggs & toast breakfast, sandwich for lunch, or chicken dinner with hawaiian rolls.
No not in the uk. I'll have it with soup but mostly we'll have a loaf in the kitchen to tear bits off in between meals as a snack with cheese or butter

I like having some dinner rolls with a roast, bread with soup but other than that unless it's a Sandwich or burger nope
I eat bread with meals usually, but I'm a diabetic, so I need the fiber. I buy the French bread every once in a while, and toast it in the oven with butter.
US, here. Not every meal. Sometimes I will have bread, sometimes I will have rice, and sometimes I will have potatoes. I like to mix it up.
Germany holds all the patents on good bread. :-D
I doubt any other nation comes close to our variety (2500+ types of bread).
I like bread but it's mostly garbage food. So I try and eat minimal gluten products.
I couldn't conceive eating it every meal.

Just about anywhere you eat here in the US they serve some form of bread i. e. bread sticks, sliced bread, hot rolls etc.
No not every meal just meals where it seems suited like soup or lasagna or spaghetti etc. I don’t eat bread with a rice dish or a potato dish or anything like that. but it’s all up to one’s discretion
Sometimes we have bread with our meal, sometimes we don't. It really depends on what we're eating. If the meal is already high in carbs, adding bread is typically not done.

I have been eating better, so I do not eat as much bread as I used to, and I really like bread.
