Yeah, I've done breakfast in bed both ways. Usually it's just if someone is feeling sick though rather than spoiling. Unless the sheets are going to be changed after, getting crumbs in bed is gross. Sheets get changed after a cold goes away, so that works out.
As for who, that would be my wife making it for me, or me making it for my wife, both of us for the kids depending on who is home with them while they're sick. Though it's dubious as to the effort involved to "make" a can of soup.
I have only had breakfast half in bed during vacations when I'm at a hotel in the morning and there is no chance to eat at the breakfast buffet, and the tables are not big enough to seat all the people in the hotel room who are eating breakfast so I prop myself on a bed. I would lay my body 90% out and have only 10% of my body propped up against the bed cause it's not just the hotel staff that will be annoyed if there is a mess, I don't want to sit on a grainy and dirty bed when I eat breakfast.
There are special bed-breakfast tables. It looks just like a tool for hospital patients who can't leave beds.
I used to make my parents breakfast in bed as a kid on the occasional weekend. The only time I've ever had it myself was ordering room service on an expense account when traveling for work, and that was maybe 3 times.
Hell, I'd be thrilled with a warm Egg McMuffin in bed these days, but I'm not holding my breath. Lol.

I tipped the serving tray over spilled my coffee all over my dick and had Third-degree burns all over my dick, got strawberry marmalade over my clothes and when the coffee fell, I tipped the whole damned tray and spilled my scrambled eggs on the floor

Toast and marmalade, corn flakes, a yoghurt (to supply one of the medications* because I can't take tablets), orange juice and coffee
* The painkillers were liquid and the only one I did take was microscopic
No I’ve never eaten in bed my whole life. I just don’t like it. It’s like a sick person and plus it’s impossible not to spill a little bit of crumbs and then they’re gonna be in your bed. Then the next thing you know you got ants and roaches crawling around on your bed. Thats not entertaining to me or being spoiled eating in bed because I’d rather go in the living room and turn the TV on and eat in the chair as I always do every night
Yes I have. Best time ever was when my young kids made breakfast for my then-wife and me on our anniversary. I think they were around 10, 8 and 4 yo. It was a treat. They made cereal and toast, with orange juice.

I've had breakfast in bed many, many times, on several different circumstances. Served by my Mom, and by several girlfriends, and my wife (s) and on many occasions by nurses when in a long-term hospital stay. It was appreciated on every occasion, love 'em
