It depends on the person who holds the door if it's a shy person then I think it was nice if it's 1 of the jerks at school I think they just wanted to get through and that they just did it without any feelings

That happened to me a couple of times and just like if another guy hold the door for me, I definitely say thanks because having good manners isn't exclusive to one gender
As to my reaction I say thanks and smile if it was my cousins or aunt's. And with my nieces once I walk through they tend to race to grab my hand and then use me as a walking giant plank to hide behind. As they are very public shy.
Don't know where you all live, but where I live people are courteous to one another. Doesn't matter who you are, we hold the door if you're behind us or a few steps away.

I've had my nieces and aunts and female cousins do this when we use to hang regularly.
