Hmm that is a bit odd that he keeps bringing up your licenses in that other country. On the one hand, it could just be innocent curiosity since you've lived/worked there before. But the repeated questions do make it seem like he's seriously considering a move.
I'd say trust your gut - if deep down it feels like he's hinting at wanting you to potentially relocate with him someday, then you're probably picking up on something real even if he hasn't come out and said it directly yet. Guys can be indirect like that sometimes.
Since he's going through family stuff now, maybe don't push the issue. But you could try casually asking what types of jobs he's been looking at, just to get a sense of if they're really in that other place long term. No need to stress him, just chat it out lightly.
My advice would be enjoy things as they are for now while being alert to any other signs he may want more from you down the road. No harm in being prepared if it does come up officially someday soon. Keep communications open and see where things lead!
Certainly sounds like he is eyeing up the future to some degree. I don't know why else he would be concerned where your medical licenses are valid otherwise. I think it is a pretty good indication that he is interested in a future with you. He probably wants to know so that he feels more comfortable when the time comes to suggest such a big step and move in your life if they are valid so you don't have to put in extra work to get licensed in that country. That would be my guess.
