There a number of reasons. Maybe she’s a genuine, nice person. Maybe she’s after him for money. We have a number of patients who are still alive even though they’re suffering simply because the wife or girlfriend wants them alive for financial reasons. It’s sad. Maybe she’s trying to weasel her way in there to get him to leave her something financially, or to be able to say she was the only one who was taking care of him when his children weren’t even there to help. Maybe they’re helping I don’t know. Maybe he’s like a father to her, never really had a father. Maybe he treated her well so she’s taking care of it.

Not enough info. Maybe she simply wants to help. Maybe she likes him. Maybe she's a gold digger after an inheritance. There is no way to know.

Does he have a will? He REALLY should have one, but most people die without one. I'd be leery if he suddenly has a new will. That doesn't mean I suspect anything. It's just a possibility that should be considered.
