For me its seriously sign of affection and appreciation. Not only for her physical features, but it allows for me to connect with her on an emotional level. If you like the guy, do as she is doing in the photo, just smile and stare back directly in his eyes.

It means you are confident and willing to make that emotional connection with him for those few seconds. If you find it hard to maintain eye contact and you it makes you feel uncomfortable... well, that means something. You need to really explore why that is.
