Coitus interruptus (the pull-out method) is the worst contraception method. Even when done properly, it's less than 50% effective in preventing pregnancy. There is often enough sperm in precum to fertilize an egg.
The bigger problem is that Nature builds men to thrust deeper just before they ejaculate, and it's very hard for a guy to overcome that innate urge. Even if he pulls out in time, his semen may end up just outside of the vagina, and there's enough liquid there for those swimming sperm to find their way into the vagina.
Use a condom... unless you really want to get pregnant. Plus, a condom is good protection against STD/STI.

If he waited too long, and is on the edge of cumming it's very difficult to have the will power to pullout in time. It 's not that he can't actually pull out, he just doesn't want to at that point.
I can only speak for myself, but you do feel it coming... No pun intended.
Of course there is a matter of pre-cum which is multiple drops that come out just from being aroused and those men don't feel in any way but they can result in pregnancy.

Hard to say, I have had mixed success with pulling out myself. So he may have been unsuccessful or it may have been intentional. Best not to rely on pulling out.
