Today, in 1914, a person dies from this approximately every 6 seconds (extrapolated).
Probably all wars. Purely because they also cause major loss of infrastructure, causing secondary deaths (for example Spanish Flu would probably have killed a lot fewer without the great war paving the way for it). Human conflict has also occured since long before humans lived long enough to die of heart disease or cancer.

Damn. I picked heart disease before i read war, and I didn't think on a global scale, so im sure i got the answer wrong. I do know that heart diease is the number one killer of Americans

But cancer and heart desease will have been around a lot longer whereas the plague disappeared thanks to hygiene and later also vaccination.
Hopefully, when you ask the question again in 5 years, Ebola will not be in the list :-)

Actually the leader in killer disease in modern age like today is cardiovascular disease AKA heart disease which happens to include heart attack so I chose B.
I voted B after thinking about it for a while. 1 in 3 deaths in America are from heart disease.
