How to keep myself motivated for losing weight?

I've tried it a few times and I always seem to get back in my older pattern. I live healthy and work out for like two weeks straight and then it slowly goes back to the way it was before. I start eating unhealthier and start skipping work outs etc. with the idea of: "I've done so good lately I think I can eat this or skip this". Are there any people that have the same problem as me and overcame them? Perhaps it's just lazyness but I really do want to lose some weight, like 20 pounds or something.

Any tips on losing weight and keeping yourself motivated are highly appreciated!
