Do leg training and eat more so that when you're gaining weight it is in muscle. If you don't like being toned just skip the upper body training but I really don't think you'll have to worry about "looking too muscular." Muscle weights more than fat so you won't see yourself bloating up as you gain muscle. Plus when you gain muscle you'll need to eat more to maintain it. Your caloric needs go way up.
Also doctors these days will prescribe anything for money. They like to bill insurance companies as much as possible. So don't think everything a doctor says is legitimate. If you don't like something a doctor wants to give you, you're under no obligation to take it no matter how important he tries to make himself sound.

Eat quality foods in good amounts
Make sure you have a small calorie surplus, some where around 300calories more than you normally consume to maintain weight

it will take time but you will get there :)peanuts, nuts, anything calorie dense. you should snack on. A healthy goal is to add 500 extra calories to your diet per day. Which will add up to 1 pound per week. Then once you stop gaining, you add another 100 per week. Remember each pound of muscle takes 18 calories a day to maintain.
Try training a bit, eat lots of carbs and protein but the key is you have to train so that you don't put it on as fat, the training is what stops it getting stored as fat without it you would gain bad weight
Avoid skipping breakfast. It is very important for gaining weight. If possible eat about six times a day. The three main meals plus snacks in between.
