I have heard that most facial exercises are not helpful. The bottomline is that you can't lose fat only in your face, you have to lose fat overall to make your face less puffed up. This makes it very difficult for thin people to lose fat on their face. I have always had a puffy face even though I have a BMI of 18.5. I am visibly very thin, people always tell me to eat more, but then my face becomes even puffier. Are there any specific weight gain and exercise regimes I can follow in order to reduce facial fat whilst also growing a more muscular physique? Or is there really nothing I can do about this?

I also tried face exorcise but with no effect... I really don't want to get surgery but it seems like there is no other way

You can't target fat like that without surgery. Lowering your body fat percentage is how you lose fat anywhere on your body.
