A bit late after the killing of the United healthcare CEO, but was just curious about this topic since I carry UHC myself for insurance.

Several years ago, an ambulance took me to the hospital for severe abdominal pains. They kept me overnight in the ER and gave me various medicines. By the next mid-day, I was only and went home.

Because the hospital didn't admit me but kept me in the ER, my HMO insurance initially refused to pay the claim, which was about $1000. It took several months of fighting them, including getting a letter from the hospital stating that they kept me in ER because no hospital beds were available. I finally did win, and they paid the claim, requiring me only to pay the $25 copay for an emergency room visit.

Shortly after that, my employer changed its health insurance company, tired of all the complaints they were getting.

One time I worked for a company that had a really crappy insurance plan. My son broke his wrist and he went to a hospital that was in their network. I thought the bill was paid. A few months alter I left the company because they moved out of state. I got a bill for the whole emergency room visit and was told that the hospital was in their network but the ER was not. Fortunately I was just starting a job as a network engineer at the same hospital and they forgave the debt.

Yes a few years ago I was denied a disability claim. The company said I had enough hrs of work but not enough months of employment. What made it so bad was it took them 2 1/2 months to make this decision.
