I just came to the realization ill be 40 in just 6 years. Im way out of shape. 5'9" and 220 pounds. I was about 145 when I was 24 and could do 24 push ups without much effort. Now I can barely do one, and i get winded from going on a 15 min walk ok level ground. Im afraid that even if I start to exercise and eat right, I've reached the point of no return, and my insides are all fucked up already, and im destined to be a cripple by the time im in my 60s. I mean, I go to the doctor often and I have high blood pressure which is controlled with meds.

Your future lies within your head. If you already think at your age that you are "done", then you will never want to do something to improve your health.
You can start by consulting a dietician and eat healthy and avoid junk food and fizzy drinks. Your future starts with a proper veggies and fruit diet and of course, some light exercises.
However, it takes a strong will do adhere to such a program. Do you have it and are you able to withstand temptations?
It does not have to happen within 3 months but losing weight and adapting your diet for long term results takes years but you are still not too old for that.

As long as you have an ok from your doctor I would start now. It means a true lifestyle change and it will not be easier as you get older. You have to lean to accept being uncomfortable during exercise or even changing your diet. Start out with baby steps and go from there. A daily walk is a good start and less carbs and processed foods and more fruits/vegetables.
Having a life coach or even a girlfriend or wife to encourage you daily to go for a walk etc. is a start. Has being out of shape affected your sex life also?

You're still young. Cut out fat and sugar. Get some exercise. Once you do that the weight will slowly drop off and you'll feel better. No candt or sugary drinks no greasy fast food. Chicken rice stuff like that whole foods.
