I accidentally voted wrong even though I read your update before voting. lol It's a completely appropriate gift - there's nothing wrong with getting him something practical like underwear, especially since you got other practical items as well. He might just need to talk to you about getting an incorrect size, or maybe he would prefer a different style or something. It most likely isn't a big deal, especially since he specifically said not to worry about it.

I think it depends on where you are physically in your relationship. If you have gotten to a point where you see each others' underwear, I don't think giving underwear as a gift is inappropriate. If you haven't gotten to that point, it might be a little presumptuous or premature. But he says not to worry, so don't! No matter what, you meant well. I voted "No".
depends on the occasion. like maybe he thought you spent too much and he is getting you a gift that isn't as big? Maybe he opened it up in front of family and felt awkward and just wants to say give warning next time... who knows? I'm pretty curious now though can you post aftterwards what it was about?
It might send him a message that you want him in bed. If that is what you want to do, then it's a great gift.

Not as much inappropiate as just weird. I have never recieved a gift from a girl (except my mom) that gave me boxers.
