Gay? Sigmond Freud had a theory that the obsession with the butthole was either and over obsession with cleanliness or an under obsession. The term "anal-retentive" comes from it.

Now anecdotally it sort of makes sense from what I've seen. The guy is either so ultra-clean and neat or he's so filthy it grosses people out.

Not trying to be offensive, make a joke, or anything like that here... just throwing what I was taught about gays in the early 80's out there.
