Unmarried: If she has the money, yes. If she's smart she wouldn't try to organize a celebration that she can't afford. If she doesn't have any money then she shouldn't be organizing anything.

Married: Yes, in the sense she would use her own bank card/cash. But what really happens is that whoever makes the most money really is paying since finances are shared. Even if bank accounts aren't shared if your husband pays 99% of the bills it amounts to the same thing in this situation. Still, the gesture/idea that she put in the effort to would be appreciated.

Thankfully I was never in a relationship with a woman who was "too good" to pay for my birthday dinner or a wife who didn't care enough to organize birthday stuff.

If my partner is planning something with me for my birthday, then it can go any way for that scenario. We could split it, I might offer to pay if I'm feeling like I can spend, or if she's feeling generous for my birthday, she'd offer to pay.

If it's me taking my partner out for my birthday, then it's on me because I'm the one suggesting to go out.
