Hmm, shaking hands with an old ex isn't too weird in that kind of casual run-in setting I don't think. Like if it was just a quick hello while out with friends, shaking hands is pretty normal and polite.

I wouldn't overthink it too much man. Chances are she was probably caught off guard seeing you after so long too and was just being friendly back. As long as you kept it brief and didn't like stare into her eyes longingly or anything creepy, I'm sure it's all good.

Old flings can be awkward to bump into sometimes. But a handshake is like the most mild contact possible haha. Unless she seems standoffish next time you see her or tells someone she thought you were a weirdo for it, I wouldn't stress too much. Water under the bridge at this point after 12 years!

If you wanna play it super safe though, just casual nod/smile from a distance going forward. Nothing wrong with being cordial but keeping your distance if she's not single or whatever. All good either way bro, just live and let live with old exes I say.
