They visit a celebrity they have never met before in person Instagram page and then write a comment insulting the celebrity physical appearance.

I assume they are probably projecting. Which means the people who insult the physical appearance of others don’t see themself as physically attractive.

It might not be the case 100 percent of the time but I believe most of the time it’s that way.
