There are 2 kinds of speculums, the ones most medical staff use and those are made of medical grade metal and there are the cheaper ones made of plastic that are disposable.
I don't have the experience to give you any feedback on those medical instruments but from what I hear, most serious doctors use the metallic ones. However, you have to ensure that your gynecologist places a high priority on hygiene and that those instruments are sterilized after each use to avoid to get infections. Ask your obgyn to see where he/she sterilizes them and see by yourself if the premises look clean.
The plastic ones usually come wrapped in individual sealed plastic wrappers and should be discarded after each use.

It might be uncomfortable at first, but it should not be actually painful beyond a second or two. Many are cold, but I always appreciate the doctors I've had who warm them up. Some warm up the lubrication gel to help as well. It may seem impossible, but it's best if you can purposely relax the area and not tense up. Of course everyone feels awkward, but it IS possible to learn to relax and just let it happen. Like someone else said, it's usually over fairly quickly and no harm done!

Cold weird thing lol It's just weird but the check up is over pretty fast and you don't have to worry
They hurt, feel like they are prying you in two if they open it too wide... but it'll be okay, just part of life. You'll survive, just like the rest of us.
It feels awkward at first, but after that it is not a problem.
